Filing a Claim is Easy

Call our toll free number, 1.877.624.4480, to inform us of the death and we will immediately verify the death benefit. We have 24-hour access for claim verification. If Funeral Directors need information at night or on the weekend, please call our 24-hour claims number, 1.800.750.4656.

We have the ability to electronically transfer claim payments directly into a provider's checking account and will call to notify you of the transferred amount. This service is the fastest means for providers to receive their funds.

A heartfelt sympathy letter, personally signed by the chairman, is also sent to the family extending condolences and letting them know that their funeral home has received payment from their policy.

How to File a Claim

To report a death claim, contact our service center at 1.877.624.4480 (Monday–Friday, 6:30 am–5:00 pm MST, or if after hours, call 1.800.750.4656.

When you call, please have the name of the deceased, the policy number(s), and the date of death on hand. Our service center will walk you through what we need for the claim and answer any questions you may have.

For most claims, we will need:

  • Copy of the insured's death certificate
  • At-Need invoice if the funeral home is the beneficiary "as interest may appear."

If your specific situation requires any additional forms, please call us. The Service Center will let you know which forms are needed, and can fax or mail them to the personal representative or to the funeral home for the convenience of the family.

You may also report a claim by mail, fax or email. If you choose to report a claim using one of these methods, please be sure to complete the claim forms if the policy is less than 2-years old, especially the representative information, so we can reach him/her if we have any questions or need additional information. When can I expect to receive the benefits?

In most cases, we issue the benefit check within 24 hours (or the following business day) of receiving the documents needed for the claim. We use standard mail so it may take about a week for the check to arrive. In some cases, we can overnight the proceeds (overnight fee will be deducted from the benefit check) or direct deposit them into an account. If you would like us to use one of these methods, please instruct us to do so when you call in the claim. We will save your account information for future claims.
